It is a requirement that parental/guardian consent is obtained from parents/legal guardians for students under the age of 18 years to be able to participate in a course and to allow (or not) personal information about the student to be used and disclosed to third parties and National Centre for Vocational Education Research Ltd (NCVER).
Before the student commences the course, the parent or legal guardian of the student who is under 18 years of age must read the Privacy Notice on the Australian College website, then complete and sign the Declaration and Disclosure Agreement form below.
Parent/Legal Guardian Declaration and Disclosure Agreement
understand that by signing this form I am declaring that:
- I give permission for the student to attend the course he/she is enrolled in, and undertake all of the requirements for the course; and
- I have ensured the student has read, understood and accepted the Terms and Conditions of Enrolment that apply to the student for the duration of the course; and
- I agree to provide Australian College Pty Ltd the required personal information for a certificate to be issued as per the Enrolment Form;
